New Patient Paperwork

In-office appointments:

If you are a new patient at Well Rested Sleep Specialists, please complete the New Patient Forms prior to your appointment.

Completing these forms in advance will facilitate a smoother visit, allowing you to arrive approximately five minutes prior to your scheduled appointment.

If you are unable to print the forms in advance, please plan to arrive 30 to 45 minutes early on the day of your appointment to complete them in the office.

Please bring your completed paperwork, insurance cards, co-pay, and any sleep disorder treatment devices such as CPAP, BiPAP, iNap, Excite OSA, or oral appliances, including any supplies associated with the devices. This will ensure that your supply order is processed promptly.

Virtual appointments:

For virtual appointments, please ensure that all necessary paperwork is emailed and received at least one day prior to the scheduled appointment. This should include a copy of both the front and back of your insurance cards. Failure to do so will result in the cancellation of your appointment.

If you are using a device such as a CPAP machine that does not have a modem for compliance monitoring, you will need to either visit the office for your appointment or bring the card to our office one day prior to your appointment. Otherwise, we will need to reschedule for a later date.

Please contact the front desk at 901-881-2260 to cancel your appointment at least 24-48 hours in advance. Failure to do so may result in a $50 cancellation or no-show fee.
