New Patient Paperwork

If you are a new patient at Well Rested Sleep Specialists, please complete the New Patient Forms and the ESS questionnaire prior to your appointment. If you use a CPAP device, kindly also complete the Remote Monitoring form. Completing these forms in advance will help streamline your visit. If you are unable to print the forms, please arrive 30 minutes early on the day of your appointment to fill them out in the office. Additionally, a fillable PDF version is available for use on your desktop ; please be sure to save the completed document and send it via email to Marjorie@wellrestedsleep.org.
